Job Summary:
The Health Records Director reports directly to the Chief Compliance Officer and provides overall leadership and day-to-day management and operation of the Health Records Department. Requirements for Health Record staffing and patient record management are outlined in the Centers for Medicare and Medicaid (CMS) Hospital Conditions of Participation (CoPs), The Joint Commission Hospital Accreditation Manual standards, and state licensing healthcare institution (HCI) rules.
This position is responsible for the overall leadership, supervision, and operation of the Health Records Department. This position provides oversight of patient health record management for the Civil and Forensic hospitals in accordance with regulatory requirements. The position is directly responsible for policy development and implementation related to patient health record management, and the safety and security of health records. The position is essential to the implementation and maintenance of the Electronic Health Record (EHR), including development of new policies and procedures for the EHR system and the Health Records Department.
As a superuser of the EHR, the Health Records Director assists staff with the proper usage of the system. This position also serves as the Hospital’s Privacy Official. In this capacity, the Health Records Director provides guidance to Hospital staff and assists in the implementation and maintenance of the Hospital’s privacy policies and procedures. The Privacy Official completes periodic compliance monitoring; works with legal counsel and management to ensure proper authorization forms and legal requirements are met; oversees and delivers privacy training to new employees; works cooperatively with Agency Privacy and IT Security staff in reporting and monitoring incidents and policy development; and monitors the release of PHI through the Health Records Department.
The Health Records Director serves as the Hospital liaison with the ADHS Office of Administrative Counsel and Rules (OCR) and Attorney General’s Office concerning requests for patient records, including public record requests, court orders, and subpoenas.
Job Duties:
− Manages the day-to-day operations of the Arizona State Hospitals Health Records Department; serves as Privacy Official for ASH; participates in ASH committees and advises ASH Executive Management of updates regarding patient health record requirements; prepares monthly, annual, and ad-hoc reports as needed.
− Conducts privacy training and orientation for new employees; conducts HIPAA rounds; receives, investigates, tracks, reports privacy and security incidents to ADHS Privacy Officer and Security Team; ensures receipt of proper authorizations prior to disclosure of PHI.
− Serves as Hospital liaison with Office of Administrative Counsel and Rules regarding patient record requests.
− Develops policies and procedures for the Health Records Department; ensures the safety, accuracy and security of patient health records; ensures all policies are in compliance with CMS, The Joint Commission, and state licensing requirements, as well as other applicable federal, state and local regulations.
− Directs, manages and supervises staff; hires and evaluates staff, addresses performance issues; approves leave; attends conferences and participates in ongoing education.

Knowledge, Skills & Abilities (KSAs):
Knowledge of:
− Knowledge of Federal, State and local laws and regulations related to health records to include the Joint Commission.
− CMS standards and regulations and State Licensure; HIPAA; health information management industry standards
− Computer hardware and software, including Microsoft Office and Google products
− Principles of management and supervision; filing systems.
− Knowledge of the performance management (PM) model and continuous quality Improvement (CQI) methods in a public health setting.
Skill In:

Retirement:

Required Qualifications
Pre-Employment Requirements:
– Must possess a valid level one fingerprint clearance card issued pursuant to A.R.S. 41-1758.07 or must apply for a level one fingerprint clearance card within 7 working days after beginning employment.
Driving is required for State Business.
– Employees who drive on state business require possession of and the ability to retain a current, valid state-issued driver’s license appropriate to the assignment.
