Job Description
This individual will conduct necessary audits of the medical record to verify that providers have appropriately documented chronic medical conditions, then code these in ICD-10 to capture HCCs and improve the organization’s MCR Risk Adjustment Score. Evaluate medical record documentation (outpatient/inpatient) from a clinical standpoint for evidence of the possibility of additional medical conditions that may not have been documented in the past. This process involves a very strong understand of medical coding and excellent communication skills.
Required Qualifications
Education: Active coding credential through AHIMA or AAPC required
License Requirements: Active coding credential through AHIMA or AAPC required
Preferred Qualifications
Preferably three years’ experience as a medical coder. Extensive knowledge of ICD-10-CM outpatient diagnosis coding guidelines (with knowledge and demonstration understanding of CMS HCC Risk Adjustment coding and data validation requirements is preferred). Ability to code using an ICD-10 CM book, even though coding would be completed through an encoder. Strong clinical skills related to chronic illness diagnosis, treatment and management. Reliability and a commitment to meet tight deadlines. After training and demonstrating knowledge and understanding of the overall position, this individual will need to have strong personal discipline to work remotely without direct supervisor. Knowledge of HIPAA, recognizing a commitment to privacy, security and confidentiality of all medical chart documentation.
