Job Description

Coder reads patient records and pertinent documents. By governmental, third party payer, and facility rules and regulations, accurately assigns and sequences ICD-10-CM diagnosis, ICD-10-PCS procedural codes, and POA indicators to inpatient records for use in reimbursement and data collection. Coder expected to code and abstract all inpatient records with a minimum accuracy rate of 95%.Coder expected to contribute to the efficiency of the HIM Department. Routinely volunteers to assist others when his or her work is completed. Perform other duties as assigned. Coder expected to prioritize job duties to align with middle revenue cycle performance goals.

Required Qualifications

Education: High School Diploma or GED

Experience: Five years of related experience.

Licenses and Certifications:

Certified Coding Specialist

Registered Health Information Technician

Registered Health Information Administrator

Qualifications:

Meets productivity requirements: Productivity requirements include assessing records online, abstracting, maintaining logs, keeping current with office email, and querying the physicians, if needed, for clarification of documentation and or additional conditions to capture the severity of illness and risk of mortality. *Inpatient: 3 on average including variances in LOS per productive paid hour. Includes ICD-10-CDM diagnosis with POA indicator and ICD-10-CM code assignment.

Communication: Excellent written and oral communication skills and the ability to work independently with minimal supervision is required. Must have working-level knowledge of the English language, including reading, writing, and speaking English.

Computer Typing: Use of computer, keyboard, and mouse, experience with basic Microsoft party payer websites for coding requirements, and medical necessity guidelines is required. Competent in accessing and using an encoder (i.e., 3M, MModal, Trucode) is required.

Knowledge: Strong knowledge if anatomy, physiology, medical terminology, ICD-10-CM coding guidelines, and reimbursement methodologies required. Courses in physiology and pharmacology preferred. Graduate of a coding certificate program or an associate or bachelor’s degree in health information technology or other allied health field is required.
