Health Information Management is responsible for the:
- Oversight of the quality, timeliness, and accuracy of the medical record and patient indexes for patient care, legal, revenue, research, and regulatory needs.
- Classification of diagnosis and procedures according to approved classification and nomenclatures such as ICD-9, CPT, Snomed, etc.
- Maintaining the security and integrity of health information.
- Providing documentation tools/services such as dictation, transcription, electronic templates, scribes, and paper forms.
- Collection, quality control, and dissemination of data for comparative data bases and statistical reports including specific disease and procedure registries.
- Providing access to medical record information through release of information processes.
The Clinical Documentation Improvement (CDI) Analyst in the Department of Health Information Management provides concurrent review of the medical record to ensure that clinical documentation supports the patient’s disease complications, severity, co-morbidities and risk of mortality.
Primary responsibilities include:
- Queries the professional staff and other caregivers as necessary via written/verbal communication to obtain accurate and complete provider documentation that supports the severity of patient illness and risk of mortality.
- Performs thorough chart reviews to identify comorbidities/complications, and documents these appropriately on the DRG worksheet.
- Demonstrates an understanding of the importance of and makes an effort to capture all potential secondary diagnoses for creating an accurate patient profile.
- Demonstrates creativity and enthusiasm while pursuing the goals of the DRG Assurance process, of the department and the organization.
- Maintains the ability to be flexible, and prioritizes daily responsibilities.
- Prefer previous Clinical Documentation Improvement experience or ten years of inpatient coding experience.
- Professional experience as a Physician, Registered Nurse, Physical Therapist, Occupational Therapist, Pharmacist, or Respiratory Therapist with ten years pediatric experience and knowledge of documentation and coding systems is also qualifying.
Knowledge, Skills, and Abilities:
- Knowledge of electronic medical records, care delivery documentation systems and related medical record documents.
- Knowledge of pediatric age-specific needs and the elements of disease processes and related procedures.
- Strong broad-based clinical knowledge and understanding of pathology/physiology.
- Excellent written and verbal communication skills and critical thinking skills.
- Ability to work independently in a time-oriented environment.
- Computer literacy and familiarity with the operation of basic office equipment.
- Assertive, respectful personality traits to facilitate ongoing provider communication.
- Ability to read medical record documentation (paper and on-line) in the performance of job responsibilities.
- Inpatient coding experience and/or advanced extensive knowledge of complex disease processes with a broad clinical experience in an inpatient pediatric setting preferred.
- Ideal candidates will be licensed as either RHIA (Registered Health Information Administrator) or RHIT (Registered Health Information Technician).
This is a 1.0 FTE (80 hours per pay period), exempt, salaried position. Compensation will be determined based on experience and market data.
