SUMMARY:
Neshaminy Manor is the 360 bed sub-acute/skilled care facility owned and operated by the County of Bucks.

Working in a spacious office, this HIM position is primarily responsible for analysis of medical record documentation, assignment of accurate ICD-10 codes, completion of portions of the MDS and maintenance of the diagnosis list, all within our electronic charting system (ECS).

Additional job diversity of QI auditing and performing other HIM functions are also expected.

Teamwork is paramount within this boutique department which consists of three HIM employees; Six Unit Secretaries are an extension of the department stationed on each of the 60 bed Nursing Units.

HEALTH INFORMATION TECHNICIAN

SUMMARY: Responsible for analysis of medical record documentation for assignment of accurate ICD10 codes. Monitors and audits chart documentation, release of information in accordance with federal and state laws and other medical record/Health Information Management (HIM) related duties as needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
• Performs analysis and coding of medical record documentation utilizing ICD-10 coding manuals. Maintains and updates current diagnosis list on resident medical charts. Queries physicians regarding diagnoses when necessary.
• Completes diagnosis information on the MDS (Minimum Data Set) for submission to the state.
• Performs discharge chart analysis and discharge procedure for closing records. Responsible for chart assembly, analysis and notification of departments with documentation deficiencies. Monitors for completeness.
• Performs additional audits as delegated by Director.
• Maintains weekly schedule for thinning charts. Returns them to HIM. Assembles and files charts.
• Responsible for Release of Information in compliance with federal and state regulations.

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. • Excellent computer skills.
• High school diploma or equivalent.
• RHIT or RHIA Credentialed or LPN/RN required.
• Background and experience in Health Information Management (HIM) practices.
• Experience with Microsoft Windows (Excel/Word) and data entry skills and familiarity with ECS Medical Records.
• Experience in chart analysis and proven skills in ICD10 coding of diagnoses/procedures.
• Solid understanding of medical terminology and demonstrated ability to accurately spell and interpret medical terms.
• Ability to maintain confidentiality.
• Mathematical abilities required for compilation of audit statistics.
• Detail oriented individual able to work independently with minimal supervision with a strong sense of team spirit.

Required Qualifications

RHIA or RHIT credentialed, or, LPN or RN required
ICD-10 coding proficiency

Compensation/Benefits

County and Union benefits equivalent to an $18,000 benefit package including pension, and medical, dental, vision. 14 paid holidays, vacation and sick time accrual. Starting salary with increase in 6 months and a COL increase in January per Union contract.
