Accountable for conversion of diagnoses and treatment procedures into codes using an international classification of diseases. Requires skill in the sequencing of diagnoses/procedures to optimize reimbursement. Ensures that records are coded in an accurate and timely manner

Job Description

  • Ensures that records are coded in a timely manner.
  • Reviews chart thoroughly to ascertain all diagnoses/procedures.
  • Queries responsible physician in a professional, tactful manner, if diagnosis is not available on chart.
  • Coder’s diagnoses and procedures on clinical summary agree with physician’s preference 100% of the time.
  • Refers chart to director, if there is a question regarding the diagnoses/codes.
  • Utilizes computerized coding/abstracting equipment.
  • Codes all diagnoses/procedures in accordance to ICD-10-CM coding principles and the Coding Manual.
  • Meets productivity standard of assigning codes at a rate set by the Director.
  • Meets quality standards of having 95% of principal diagnoses and procedures appropriately and/or correctly coded.
  • Maintains 99% rate of information correctly abstracted.
  • Reviews coding periodicals within 7 days of receipt.
  • Notifies director whenever work is more than 48 hours behind work deadline.
  • Assists the Director with state requirements and reports.


  • Must be familiar with all charting and coding requirements, including pediatric and SNF.
  • Maintains a good working relationship within the department, other departments and medical staff.
  • Demonstrates knowledge of Release of Information requirements/regulations.
  • Willing to accept additional assignments.
  • Performs performance improvement functions through data collection and documentation review.
  • Ensures data quality and optimum reimbursement allowable under the federal and state payment systems.
  • Acts as a resource person to hospital staff for coding and may provide educations regarding coding changes/issues.
  • Ensure chart is complete and up to date at all times.

Required Qualifications

  • Ability to work with physicians in a collaborative manner.
  • Computer knowledge
  • Ability to read and communicate effectively in English.

Preferred Qualifications

  • Two or more years of previous hospital experience as a Coder preferred.

Education Qualifications

  • High School graduate or equivalent.
  • Knowledge of diagnoses/procedures in accordance with ICD-10-CM coding principles for both acute and skilled nursing facilities.
