Job Description

Performs concurrent qualitative and quantitative analysis of medical records (assembly/analysis) for completeness, accuracy and conformance to hospital accreditation standards; Maintains HMS database of incomplete/delinquent records.
Maintains all necessary medical records in a timely manner. Works with medical staff to achieve timely completion of medical records and communicates incomplete and delinquent record notices to physicians at least weekly. Facilitates timely completion of documentation queries with the medical staff to support coding and billing functions. Scans medical records and files all loose reports and transcribed reports daily without backlog.
Creates and implements contingency plans and procedures to facilitate ongoing records management in the event of potential physical damage to records. Trains backup staff and assures coverage of functions. Actively participates in PPS and Outlier meetings related to documentation improvement issues. Manages functions related to the release of confidential medical information to authorized requestors in accordance with established guidelines. Maintains release of information log.
Monitors admissions daily for compliance with requirements for complete/updated H&P on chart within 24 hours of admission.
Performs chart reviews and data collection, including collection of consultants with accuracy per policy.
Ensures appropriate processes for maintaining current appointments, reappointments and changes in Medical Staff privileges and delineations.
Coordinates data collection and profiling of clinical competency in adherence to Medical Staff Bylaws.
Coordinates completion of credential files for each member of the Medical Staff and applicable non-physician individuals with delineated practice responsibilities.
Participates and assists in medical staff performance improvement processes.
Maintains all medical staff credential files and related peer review activities with strict confidentiality. Ensures continuous review and updating of credential files regarding current licensure and certification, verification of data bank and other requirements. Maintains accurate and complete credentialing files in accordance with Bylaws, JCAHO, CMS, state and other requirements. Prepares credential files and other credentialing data for Medical Executive Committee and Governing Board review. Maintains regular communication with Credentialing Verification Organization (CVO) to obtain updated credentialing information, if applicable.
Abstracts clinical and quality data from medical records for internal and external reporting needs.   Works with the leadership team to ensure timely collection and submission of data, including CMS reporting requirements.
Meets annual goals as established with supervisor.
Performs other duties as assigned.

Required Qualifications

• Requires Current credentials as a Registered Health Information Technician (RHIT) or RHIT-eligible status, Registered Health Information Administrator (RHIA) or RHIA-eligible status, as required by state regulation.
• One to three (1-3) years experience in a Health Information or Credentialing Department preferred.
• Working knowledge of medical staff organization and credentialing activities are needed.

Education Qualifications

Requires an Associate’s degree in a health care related field.
